- Download the completed application and save it to a local computer or device
- Follow the payment portal link and make payment for plan review.
- Download payment receipt and save to local computer or device. (Be advised if payment is not made and included with submittal, the documents will be rejected and a new submittal will be required)
- Follow the link for the applicable checklist (No checklist required if submittal is a supplement to a previously approved plan)
- Complete the information on the checklist, download, and save to local computer or device
- Return to this page, fill out the fields below and attach files.
