Plan Review Portal - Supplements

INSTRUCTIONS

  1. Follow the link to the Plan Review Application in the right column and complete the information on the application.
  2. Download the completed application and save it to a local computer or device
  3. Follow the payment portal link and make payment for plan review.
  4. Download payment receipt and save to local computer or device. (Be advised if payment is not made and included with the submittal, the documents will be rejected and a new submittal will be required)
  5. Return to this page, fill out the fields below, and attach files.
