Online Customer Service and Alternatives
ADOH offers much information on-line that you might otherwise have to collect in person. Our goal is to provide citizens with a more convenient and efficient means in which to interact with Arizona government. If at any time you choose not to proceed with a transaction offered through this web site, your choice will in no way affect your eligibility to receive benefits or services. If you prefer to apply for benefits or services in person, you may do so at the respective State agency providing such benefits and/or services.
Collection and Use of Personal and Financial Information
Information You Provide
For specific programs applicants directly or through an agent of ADOH may submit personally identifying and financial information to ADOH as part of an application for funding. ADOH only collects information necessary to administer its funding programs. Personally identifying information may include the applicant’s name, address, date of birth, and social security number, and a credit card number, or in the case of applicants that are corporations or other legal entities, a federal tax identification number. Financial information may include income information such as salaries and business income, the sources of such income, descriptions of debts and liabilities including names of lenders and principal amounts loans, interest rates, and periodic payment amounts and credit reports. Business applicants may be required to provide financial statements.
Collection of the information described above is required as a part of the program qualification and determination process and to monitor program status. This information can and will be shared with federal agencies as well as the mortgage beneficiaries and their representatives as deemed appropriate.
When ADOH requires an applicant to divulge his or her social security number, the website service will identify as specifically as possible the legal authority for the requirement.
Failure to supply the required information may disqualify an applicant for a funding program. In such cases the website will notify the applicant that providing the requested information is a condition of program eligibility and inform the applicant how to submit the information in person or by alternate means. ADOH administers some programs solely through a web-based interface. In such cases information provided to ADOH in person must be entered through the web-based interface by ADOH staff or its agents.
The website may also reference common infrastructure utilities that are provided by ADOH. These utilities provide the ADOH with a means to obtain specific information or to facilitate a specific functional requirement of the online transaction being requested.
Not all ADOH online applications use the website services, nor are they required to. Please be assured that your personal information will be held in strictest confidence while ADOH processes your online transaction and will not be shared, sold, or otherwise disseminated to any outside person or entity, except as required or authorized by law.
Keeping and Disclosure of Information Gathered
Information collected from an application submitted online will be treated in the same manner as the information collected from an application submitted on a hard copy to the applicable State agency.
The information you disclose to the agency (whether through ADOH's website or by other means) will generally become a matter of public record pursuant to A. R. S. §§ 39-121, et seq., which means that the agency may be required to disclose the information in response to a public records request or in response to a warrant, subpoena, or other court order. ADOH must disclose all information that is not otherwise prohibited from disclosure by statute, court order, or applicable program regulations.
Because U.S. Department of Housing and Urban Development or other federal agencies may be the source of program funding, ADOH may share personally identifying and financial information as required by applicable program regulations. In addition, some programs may require ADOH to share personally identifying and financial information with commercial lenders.
Third-party organizations contracting with the ADOH may provide certain services available on ADOH’s behalf. Personal information collected through the web portal and/or over-the-counter may be provided to third-party service providers to help the state deliver benefits and services.
Information Collected and Stored Automatically
ADOH collects personally identifying and financial information directly. With one exception ADOH does not use "cookies" to permanently gather information during your visit. ADOH uses permanent cookies solely for the purpose of knowing how many people visited the site, where they went on the site and how long they stayed on the site. Otherwise, ADOH uses “session” cookies to facilitate program functions such as registration that are not retained after your visit. Individual web sites you access through the portal may use "cookies", however, and are required to define their use in the separate privacy policy statement located on the specific site. The web server that hosts ADOH does gather certain information about the visit automatically in a standard log file. This information never identifies you personally and consists of only the following items:
- The Internet domain (for example, "xcompany.com" if you use a private Internet access account, or "yourschool.edu" if you connect from a university's domain) from which you access the website;
- The IP address (an IP address is a number that is automatically assigned to your computer whenever you are surfing the Web) from which you access the website; and
- The date and time of your access.
We use this information to gather statistics about the amount of use the site receives. Again, we do not automatically record information about you as an individual. This information is not shared, sold, or otherwise disseminated by ADOH.
Although ADOH does not currently use cookies, future releases may necessitate their use along with other session enhancement methods. These will be thoroughly evaluated in order to determine their benefit to both the client and the service provider before being implemented. This privacy statement will also be updated to describe exactly what information is being collected and how to disable or refuse cookies.
Access/Accuracy
To the extent that you have provided personal information, the State of Arizona needs to maintain accurate information about you and the benefits/services that you may be applying for and/or personal inquiries. This state agency provides a means of contact on its web site either by email or phone should you need to update or correct such personal information. The state will make reasonable and prudent efforts to incorporate your changes as soon as possible.
Information Collected from Children
By statute, all State agencies must comply with the Children’s Online Privacy Policy Protection Act of 1998 ("COPPA"), requiring parental consent before collecting any personal information from any individual under the age of 13. The text of COPPA is available at http://www.ftc.gov/ogc/coppa1.htm.
Systems and Network Security
ADOH has taken several steps to safeguard the integrity of its computer systems and networks. These steps include, but are not limited to individual authentication, the security of transmitted data, security monitoring, auditing, and encryption of your information. Security methods and measures have been integrated into the design, implementation and day-to-day practices of the entire website. The state will take prudent steps to ensure that third-party contractors are obligated to secure and protect personal information on the behalf of ADOH.

Notifications to the Public
In the event of a security breach of personal information and after relevant law enforcement agencies have determined that notifications to the public will not impede a criminal investigation or threaten state and/or national security, ADOH will notify its applicants and recipients in the most expeditious manner possible (i.e., first class mail and/or email) that is in compliance with federal and state laws.
