As the sole IPIA for the state of Arizona, the Arizona Department of Housing (ADOH) is responsible for issuing HUD labels to Manufacturers of HUD program manufactured housing. HUD labels are issued for new construction, to certified manufacturers, only. If labels on existing homes are lost or damaged they are gone forever. Replacement labels will not be issued.
HUD Labels are ordered through HUD's monitoring contractor and issued by ADOH. No more than a two-to-four week supply of labels may be ordered at a time. A fee of thirty nine dollars ($39.00) is charged by HUD for each label, in addition a handling fee of five dollars ($5.00) is charged by ADOH for each label.
The manufacturer's assigned label control personnel will initiate and complete the first section of the HUD-301 - Request and Payment for Labels at https://pay.gov/paygov/. The fee is payable with the label request. This payment is made to HUD through Pay.gov. The required payment amount must be for the number of labels multiplied by the amount of the label fee per transportable section of each manufactured housing unit. Manufacturers are to complete all sections at the top section of the HUD-301. The payment section is only for the payment to HUD for labels being purchased (and is currently not displayed at Pay.gov). The IPIA (OMH) is requested to assure that this is the correct amount. If there are credits due, they should be subtracted from the Form 303 credits on the 2nd line and a copy to the 301 attached. The actual amount of the payment to HUD is listed on the 3rd line. A copy of the completed Form HUD-301 and on-line payment receipt is mailed, with payment of the IPIA handling fee, to the IPIA. The IPIA completes Form-301 by documenting the label numbers it has assigned to the manufacturer and forwards this completed form and a copy of the receipt to HUD’s contractor. A copy of the completed Form HUD-301 is to be maintained by both the manufacturer and IPIA. A monthly reconciliation is conducted by HUD’s Contracting Agent of the IPIA’s completed copy of Form HUD-301 and the fees received at Pay.gov.
Once the home manufacturer receives their label order from the IPIA, these labels are required to be stored at the plant under strict security and inventory control. They should only be accessible to the manufacturer’s assigned personnel and IPIA inspector. When labels are being withheld by the IPIA, only the IPIA is permitted access to the labels assigned to the manufacturing plant.

Manufacturers are required, by HUD, to submit a monthly report (302 Report) to HUD through IBTS. Please visit http://weblabels.ibts.org for instructions for filing the monthly 302 report.
Manufacturers are required to report production activity to ADOH monthly. This consists of a copy of the 302 Report, including copies of the data plates for homes reported. This report must be filed with ADOH no later than the 5th of each month.

HUD LABEL VERIFICATION
In an effort to help consumers, assessors, and finance companies obtain correct information on manufactured homes, this Office has the ability to verify HUD manufactured homes produced from June 15, 1976 to present. The state can only verify production information for manufactured homes built within the State of Arizona. Mobile homes built prior to June 15, 1976 or mobile homes still on the production line after June 15, 1976, which were not built to HUD's federal construction standards, cannot be verified.
The service this Office provides is valuable to the public in obtaining correct information on homes, which may be relocated or altered in a way that may be difficult to identify original markings. Arizona has multiple environments, which dictates that some areas of the state impose more strict zoning codes and that the public obtain information about a HUD manufactured home prior to its purchase or relocation.

Understanding the HUD serial number
This Office only has the ability to verify homes with the above HUD label marked ARZ. Please contact other state jurisdictions, third party inspection agencies or IBTS for verification of their homes. Currently, the Institute for Building Technology and Safety (IBTS) imposes a fee to request a HUD verification letter, which provides HUD label numbers, serial number, manufacturer, date of manufacture, and the original dealer from June 1976 to present.
