Fraud Alerts submission form

You may report suspected housing fraud through the Arizona Department of Housing’s Fraud Alert form below. If you wish to remain anonymous, please do not provide any identifying information within the form, as any information provided may be considered a public record subject to public disclosure in accordance with State Public Records laws. If you are reporting potential fraud at a specific address, please provide the full address, including the name of rental properties (if known), and the city in which they are located. The Arizona Department of Housing will investigate any matters over which it has jurisdiction, and for those matters that it does not, it will attempt to provide the information to the proper authority for review. 

Please note: The Arizona Department of Housing does not process the following types of fraud alerts:

  • Criminal and/or Potentially Dangerous Activity
  • Scams Involving Rental Property
  • Other State Agencies (e.g. DCS, TANF, EBT)
  • Conventional Rental Property
  • Rental Properties without Federal/State Funding and/or Rental Assistance
  • Disputes involving third parties
  • Suits brought by residents against owners/agents or vice versa
  • Eviction matters
  • Interpretations of local laws and ordinances
  • Interpretations of any applicable codes
  • Disputes between LIHTC properties and Residents (pertaining to lease provisions and/or disagreements)
  • Directly involving any Public Housing Authority other than Yavapai County (Arizona Public Housing Authority)

Due to numerous privacy requirements, statutes, and laws (e.g. Privacy Act of 1974, Title 18, Section 1001 of the U.S. Code, etc.), ADOH cannot confirm or deny a review or processing will be initiated by the submission of this form, nor will responses, updates, or outcomes be disclosed. If you provide any contact information, it may be conveyed to an Owner/Agent/Entity (if deemed applicable). In turn, an Owner/Agent/Entity may use the information for contact and in the pursuit of a resolution. Additionally, if you believe you were the victim of identity theft, we encourage you to go to the Federal Trade Commission (FTC) identity theft website, identitytheft.gov to report and protect yourself from further victimization.
